Cortical Thinning Mediates the Correlation between Periventricular White Matter Hyperintensities and Cognitive Impairments

چکیده
Introduction: Previous studies showed that white matter lesions (WMLs) were an important risk factor for cognitive impairment, but the mechanisms whereby WMLs induced cognitive impairment have not been clarified. We hypothesised that the correlation between WMLs and cognitive impairments would be mediated by cortical thinning. Methods: Patients with periventricular white matter hyperintensities (PWMHs) on MRI were selected as research subjects. We investigated the contributions of PWMHs to several domains of cognitive impairment and the topography of cortical thinning and then investigated the relationship among diffusion tensor imaging (DTI) measurements, cortical thinning, and cognitive impairments. Participants included 16 strokeand dementia-free subjects with PWMHs on magnetic resonance imaging (MRI) and 20 healthy control subjects. All participants underwent an examination of cognition, cortical thickness, and a DTI scan. Results: After accounting for age, sex, years of education, and treatable cardiovascular risk factors related to cognitive performance, DTI measurements of periventricular WMLs were associated with cognitive impairment in executive function and verbal fluency and with cortical thinning in the frontal pole, orbitofrontal cortex, superior and middle frontal gyri, superior and middle temporal gyri, insula, lingual gyrus, and cuneus. Cortical thinning, but not PWMH, was independently associated with cognitive impairment. Conclusion: Our results suggest that the correlation between PWMHs and cognitive impairments is mediated by cortical thinning.

Cognitive and physiologic correlates of subclinical structural brain disease in elderly healthy control subjects.
CONTEXT Healthy elderly persons commonly show 4 types of change in brain structure-cortical atrophy, central atrophy, deep white-matter hyperintensities, and periventricular hyperintensities-as forms of subclinical structural brain disease (SSBD). OBJECTIVES To characterize the volumes of SSBD present with aging and to determine the associations of SSBD, physiology, and cognitive function. ...
متن کاملPrevalence of subcortical vascular lesions and association with executive function in mild cognitive impairment subtypes.
BACKGROUND AND PURPOSE Subcortical hyperintensities (SH) have not been systematically evaluated in mild cognitive impairment (MCI). We sought to describe their frequency and distribution, and to test their association with cognitive characteristics in MCI patients. METHODS We performed standardized neuropsychological tests and an MRI scan in 170 consecutive MCI patients. Medial temporal lobe ...
متن کاملMorphologic, distributional, volumetric, and intensity characterization of periventricular hyperintensities.
BACKGROUND AND PURPOSE White matter hyperintensities are characteristic of old age and identifiable on FLAIR and T2-weighted MR imaging. They are typically separated into periventricular or deep categories. It is unclear whether the innermost segment of periventricular white matter hyperintensities is truly abnormal or is imaging artifacts. MATERIALS AND METHODS We used FLAIR MR imaging from ...
متن کاملHigh prevalence of white matter hyperintensities in normal aging: relation to blood pressure and cognition.
The occurrence of cerebral white matter hyperintensities (WMHs), and their associations with blood pressure, episodic memory, and other cognitive tasks, were examined in a population-based sample of 123 individuals between 64 and 74 years old. Magnetic Resonance Imaging (MRI) detected subcortical and periventricular hyperintensities in 90% and 67% of the cases, respectively. Subcortical WMHs we...
متن کاملPredicting progression to dementia in elderly subjects with mild cognitive impairment using both cognitive and neuroimaging predictors.
The objective of this work was to assess the predictive accuracy of targeted neuroimaging and neuropsychological measures for the detection of incipient dementia in individuals with mild cognitive impairment (MCI), and to examine the potential benefit of combining both classes of measures. Baseline MRI measures included hippocampal volume, cortical thickness, and white matter hyperintensities. ...
